To successfully verify your identity to prescribe controlled or non-controlled drugs, you will need a smartphone or tablet, and photo ID (driver's license, passport, passport card, or state ID card).
Be prepared to provide your:
- First Name
- Last Name
- Mobile Phone Number
- Email Address
- Social Security Number
- NPI Number
- DEA Number with Schedule(s) of drugs (if required)
If identity verification is unsuccessful, it means we were unable to match the information you provided with the electronic records we use for verification. After your unsuccessful verification, look for the button that says Verify identity on a video call, and follow the prompts to verify your identity with one of our Trusted Referees.
