To be valid, all documents must clearly indicate your full name, your occupational title, and show employment for the current school year.
- Classroom Teachers can use: a state-issued educator certification, a teacher ID card, or a verification letter from a school administrator (on official letterhead, signed by an administrator).
- Principals & Assistant Principals can use: a state-issued educator certification, a school staff ID card, or a verification letter from a school administrator (on official letterhead, signed by an administrator).
- School Employees can use: a state-issued educator certification, a school staff ID card, or a verification letter from a school administrator (on official letterhead, signed by an administrator).
- College / University Professors can use: a teacher ID card, or a verification letter from a school administrator (on official letterhead, signed by an administrator).
NOTE: If your name has recently changed, and the name you submit does not match the name in your document, please provide one of the above documents and a document that shows your name change (for example, a marriage certificate or legal name change document). Combine both documents into one file for upload.
