The California Department of Public Health (CDPH) has partnered with ID.me to provide identity verification for online CDPH health record requests. If the information provided to the California Immunization Registry is incorrect or incomplete, you will need to verify your identity with ID.me to access your Digital COVID-19 Vaccine Record.
Steps to verify for CDPH:
1. Go to the CDPH “My Turn” Virtual Assistant, select a language. Read and agree to the privacy statement shown. |

2. Answer the questions asked by the Virtual Assistant, select an issue that reflects your situation, and enter your personal information.If the assistant is able to access your CDPH records right away, you do not need to verify your identity. If the assistant is unable to access your records, you will be asked to verify your identity with ID.me. |

3. Select the ID.me Create an account button to begin the identity verification process. |

4. Sign in to your existing ID.me account, or create a new one.If you have an existing ID.me account, please use that same login. ID.me only allows one account per member. |
|
|
5. Confirm your email address.ID.me will send you an email, select the link provided to confirm your email address. If you have not received an email, please check your spam and junk folders. |

|
6. Select a Multi-Factor Authentication option (MFA).The most common method is a six-digit code that will be sent to your phone number. |
|
|
7. Choose how to verify your identity.You may choose to verify your identity with a Driver’s License, State ID, Passport, or Passport Card. You can take a photo with your mobile device or upload a digital copy from your computer. |

8. If you choose to take a photo with your mobile device, follow the onscreen prompts.
You'll be asked to input your phone number. ID.me will send you a text message with a link to your smartphone to take photos of your ID. If you do not have a mobile device with a camera, you may upload a digital copy of your document. |

9. Enter all other requested information, including your Social Security number.ID.me asks for your Social Security number to prevent creating duplicate accounts. |
|
|
10. Review and confirm to make sure all your information is accurate, then select Yes at the bottom of the screen.After ID.me verifies your identity, you will receive a confirmation text message as a way to protect you from fraud. |

11. Select Allow and continue to finish your identity verification process.This step shares verification of your identity with CDPH so you can complete the submission of your remediation request. Once you have successfully verified your identity, remediation of your record will be completed by CDPH within 1 week. |

What is an ID.me Trusted Referee?
If you are having difficulty with verifying your identity online, you may be directed to meet with a Trusted Referee on a video call. A Trusted Referee is a trained identity specialist employed by ID.me to prove your identity.
When verifying your identity with a Trusted Referee, you will be asked to upload photos of your identification documents and show the physical copies to an ID.me Trusted Referee. You may need to show either two primary documents or one primary document and two secondary documents and/or proof of a Social Security number.
