If you were unable to access your COVID-19 Digital Vaccine Record via the DC Health website, it’s possible DC Health has your record, but doesn’t have your correct information. This article explains what information could be incorrect and how to request a review from DC Health.
The types of information that may not match between what you entered and what DC Health has maybe include your:
- Date of birth
- Mobile phone number
- Email address
Request a review
If the information doesn’t match, you can request a review of your record through the DC Health Virtual Assistant. From there, you will be asked to verify your identity with ID.me.
To request a review, go to the DC Health Virtual Assistant, select Let us help, then select Continue. Follow the prompts and agree to the Privacy Statement.
