The Department of Veterans Affairs (VA) is working with ID.me to let people with Power of Attorney assist veterans through the identity verification process.
Apply for a veteran with Power of Attorney:
- Verify your identity, and the identity of the veteran you are assisting, with ID.me for the VA.
Note: To verify your identity, you will be asked to provide personal information including your Social Security Number. -
Fill out and submit VA Form 10-0137 for you and the veteran you are helping get verified.
Use VA Form 10-0137 to give specific people permission to make health care decisions for you, and to let VA health care providers know your wishes for medical, mental health, long-term, and other types of care. -
Once your identity has been verified, and VA has processed the VA Form 10-0137 you submitted, you can perform transactions with VA on behalf of the Veteran you have Power of Attorney for.
