When verifying your employee or hospital employee status, you can use your work email address or upload documentation. This article lists the documents you can upload to prove your employee and hospital employee status.
Note: If you are a government employee, you will need to take different steps to verify your status. Documents you use to verify are also slightly different.
Verify with employee email address
You can choose to enter your work email address, instead of uploading a document. If you don’t have a work email address, select Upload employment documentation.
Acceptable documents
The document you upload can be one of the following mentioned below.
Certificate of employment or offer letter
Your certificate or letter must be on official letterhead and include:
- Full name
- An issued date within the last four years
- An administrator’s signature
Company ID card
Your company ID card must include:
- At least one name (first or last) and one initial (Jane Doe, J. Doe, Jane D.)
- Name of the company you work for
- An issue date, as long as it’s not expired.
Pay stub
Your pay stub must include:
- Full name
- Name of company you work for
- Dated within the last four years
Unacceptable documents
Unacceptable documents include:
- Expired IDs
- Federal ID cards from any federal department or agency
