When verifying your employee status, you can use your work email address or upload documentation. This article lists the documents you can upload to prove your employee status.
Email address
You can enter your work email address, instead of uploading a document. If you don’t have a work email address, select Upload employment documentation when prompted.
Acceptable documents
The document you upload can be one of the following mentioned below.
Certificate of employment or offer letter
Your certificate or letter must be on official letterhead and include:
- Your full name
- An issue date within the last four years for hospital employees and employees of participating organizations. For government employees, it must be dated within the last year.
- An administrator’s signature
Company ID card
Your company ID card must include:
- At least one name (first or last) and one initial (Jane Doe, J. Doe, Jane D.)
- Name of the company you work for
- An issue date, as long as it’s not expired.
Pay stub
Your pay stub must include:
- Your full name
- Name of company or government agency you work for
- Dated within the last four years for hospital employees and employees of participating organizations. For government employees, it must be dated within the last year.
Notification of personnel action (government employees only)
- Your full name
- Name and location of the organization, department, or agency you work for
- Dated within the last year
Unacceptable documents
Unacceptable documents include:
- Expired IDs
- Federal ID cards from any federal department or agency
- Notification of personnel action documents indicating you were terminated or separated from your position.
