To verify your medical status you must upload specific documents and have an eligible medical job title. This article explains what documents medical providers can upload during the verification process.
Documents requirements
Document requirements depend on your job title and the document you decide to submit. Expired IDs and documents are not accepted. In general, these documents are acceptable:
Employment ID card
An employment ID card must have:
- Your full first or last name. An initial for your first or last name is acceptable.
- A qualifying job title or position.
- An issued date within the last four years if an issue date is present. ID cards without issued dates are acceptable.
Medical license or certificate
A medical license or certificate must have:
- First and last name. Your full name must match the name you enter during verification.
- A qualifying job title or position.
- An issued date within the last four years if an issue date is present. ID cards without issued dates are acceptable.
Employment letter
An employment letter must have:
- First and last name. Your full name must match the name you enter during verification.
- A qualifying job title or position.
- A date within the last four years.
- An administrator signature. Digital signatures are acceptable.
- An official letterhead.
Paystub
A pay stub must have:
- First and last name must match.
- A qualifying job title or position.
- Institution’s name must be present.
- Pay date within the last year.
Unacceptable documents
The following documents aren’t accepted:
- Expired documents.
- Paper IDs and federal ID cards issued by any federal department or agency are not acceptable.
