What if I am unable to verify my medical provider status?

If we were unable to verify your medical provider status using the credentials you entered, your screen will look like this.

Complete the form below when it is displayed. Enter the "Email used to register with ID.me," and (if applicable) attach a photo of your DEA certificate. Then select Submit.
