To verify your status as a Hospital Employee, select one of the options below.
Use an employee email address
Use this option if you have immediate access to a hospital employee email account.
After you click Select above, select your hospital from the Employer Name list shown below. (Or click I don't see my employer listed here to add your hospital to our database. To learn more about other types of Employee verification, click here.)
After you submit your information, a verification link will be sent to the your email address. Once you receive that link and click on it (or copy and paste the manual link into your browser) your account will be verified.
Upload employment documentation
Submit a digital (scanned) copy of one of the following documents:
- ID Card (showing at least one name and one initial; for example, John Smith, J. Smith, and John S. would all be acceptable)
- Offer Letter
- Certificate of Employment
- Paystub
NOTE: We do not accept business cards or pictures of individuals dressed in medical clothing as a form of verification.
