If the name on your identity documents is different from the name you provided on your application, you may be asked to submit a name change document that shows the connection between the name on your secondary document and your current name and verify your identity on a video call with a Trusted Referee, also known as a Video Chat Agent.
1. Submit a primary document with your current name.
Primary documents must show your current name; name change documents are not accepted.
If you are verifying your identity for unemployment insurance and a primary document does not show your current name, please contact your state's workforce agency for next steps.
2. Submit a secondary document along with a name change document.
Secondary documents may show your previous name with an original or certified copy of the following linking documents that show the connection between your current name and name on the document.
Linking documents include:
- Marriage Certificate or Domestic Partnership document.
- Dissolution of Marriage document.
- Name change document.
- Adoption document.
If your name has changed more than once, a separate name change document is required for each event. Name change documents may not be used as primary or as proof of your Social Security number.
Note: If you recently changed your name, but didn’t register it with state or federal government offices, it may prevent you from accessing online services.
