The National Practitioner Data Bank (NPDB) works with ID.me to provide authentication and identity verification through the Health and Human Services (HHS) External User Management System (XMS).
This article explains how to use ID.me to request an NPDB Self-Query for yourself or your organization. A Self-Query is a search for your information or your organization's information in the NPDB.
Get started
How you begin depends on whether you already have an ID.me Wallet or need to create one for the first time. Select the appropriate tab below for step-by-step instructions.
Already have an ID.me Wallet?
Do not create another wallet if you already have one. Instead, you’ll need to:
- Add your work email
- Sign in to the NPDB Self-Query and consent to share your verified identity information
Add your work email address
To add your work email address:
- Go to ID.me My Wallet and sign in.
- From My Wallet, select Profile, then in the Email section, select Add an email.
- Enter your work email address. If you want to label this email address as your work email, check the box next to "Work".

Select Send confirmation. A confirmation message will be sent to your work email.
Tip: If you don't receive the email, check your spam folder. You can select Resend confirmation to send a new confirmation email. - Select the link in the email to confirm this email address.
After you have confirmed the email, it displays in the “Profile” tab.
Sign in and share your information
To sign in and share your information:
- Go to the Self-Query sign-in page, then select Sign in with ID.me.
- Sign in and complete multi-factor authentication (MFA).
- If required, follow the prompts to provide any additional information.
- From the Authorize screen, review the information that HHS XMS will receive, then select Allow to consent to share that information with them.
- Return to the NPDB view or order a Self-Query.
- On the “Your Self-Query Orders” page, select Start a new order to request a Self-Query. Follow the prompts to enter your order information.
Creating an ID.me Wallet for the first time?
If you have never created an ID.me Wallet, you need to:
- Create one using your personal email address
- Add your work email
- Access NPDB
- Verify your identity
Create an ID.me Wallet
To help ensure you always have access to your ID.me Wallet, create it using your personal email address, then add your work email.
To create a new wallet:
- Go to ID.me My Wallet and select Create a wallet.
- Enter your personal email address and a strong password, then select Create wallet. Learn how to create a strong password.
- Follow the prompts to finish setting up your wallet and to confirm ownership of your email.
Add your work email
To add your work email:
- From My Wallet, select Profile, then in the Email section, select +Add an email.
- Enter your work email address. If you want to label this email address as your work email, check the box next to "Work".

Select Send confirmation. A confirmation message will be sent to your work email address.
Tip: If you don’t receive the email, check your spam folder. You can select Resend confirmation to send a new confirmation email. - Select the link in the email to confirm this email address.
After you have confirmed the email, it displays in the “Profile” tab.
Access NPDB
To access NPDB, go to the Self-Query sign-in page, then select Sign in with ID.me and follow the prompts to verify your identity.
Verify your identity
To verify your identity for NPDB, use one of the following methods:
Self-Service
Upload a driver’s license, state ID, passport, or passport card and then take a selfie. This self-directed process guides you through each step on your screen, allowing you to complete your identity verification independently and at your own pace.

Video Call
Upload a government-issued photo ID, enter your personal information, then join a video call with a live agent. The agent then compares you to your identity document.

In Person
Book an appointment, then take your identity documents and QR code to a retail location to complete your identity verification.
